How to give us your feedback: Complaints and Suggestions
Do you have a suggestion or a complaint about our programs and services? We welcome your feedback.
Your suggestions and complaints are important to us: They help us to improve our services. If you are not satisfied with our services, please let us know.
To make a Complaint or Suggestion to Regent Park Community Health Centre:
- You can ask any staff person to help you make the complaint. You can ask for help in person at any of our locations or contact us by phone.
- You can make your complaint directly to one of our managers or directors. To speak to a manager or director, please ask at the reception desk.
- You can fill out a complaint form. You can pick up the form at the Reception desk or use the link to the form on the Regent Park Community Health Centre website. The link to the form is on the Contact page at https://www.regentparkchc.org/contact
Return your finished complaint form to the reception desk or send it by email to firstname.lastname@example.org
We try to respond to complaints within 10 working days.
