Intake and Client Support Worker
We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services.
Job Summary
The Intake and Client Support Worker plays a pivotal role in ensuring compassionate, responsive, and efficient care for individuals seeking support. This position is designed to engage clients, facilitate their navigation through services, and promote holistic care tailored to individual needs. The Intake and Client Support Worker works closely with a diverse, interdisciplinary team to ensure clients receive coordinated and comprehensive care in line with RPCHC’s mission of promoting health equity and community empowerment.
Permanent Full Time position – 35 hours per week, plus Benefits including HOOPP Pension
This is a bargaining unit position with OPSEU Local 5115.

Responsibilities Include
- Client Intake and Engagement:
- Conduct intake interviews with new and returning clients to gather personal, demographic information.
- Provide a welcoming, respectful, and confidential environment for clients to share their needs and concerns.
Service Navigation and Referrals:
- Assist clients in navigating the range of health and community services offered at RPCHC and beyond, ensuring that clients are aware of available resources and how to access them.
- Provide clear information on health services, eligibility, and the application process for various community support programs.
- Book initial appointments, and monitor incoming correspondence, client communication, and accommodation needs.
Crisis Management and Conflict De-escalation:
- Respond to clients in crisis situations, applying de-escalation techniques to manage conflict and reduce distress, and contribute to a calm and safe environment for both clients and staff.
- Document and report any incidents involving crisis situations in accordance with RPCHC policies to support continuous improvement and safety.
Documentation and Administration:
- Register new clients in electronic medical record (EMR) system PS Suite.
- Maintain program wait lists and produce summary reports
- Maintain accurate and up-to-date records of client interactions, services provided, and progress made.
Qualifications and Skills Required
- College diploma or university degree in social services, community health, or a related field.
- At least 3 years of experience working in community-based settings doing client intake, case management, or social services role.
- Strong interpersonal and communication skills, with the ability to build rapport and trust with clients from various cultural, age and economic backgrounds.
- Experience working with individuals facing mental health challenges, addictions, homelessness, and other barriers to health and well-being.
- Awareness of and sensitivity to the health issues of a diverse community and experience working with disenfranchised populations.
- Understanding of impacts of systemic oppression on marginalized individuals and communities.
- Ability to work collaboratively in a team-oriented, client-centred environment.
- Understanding and commitment to health equity, anti-racism, social justice, trauma-informed practice, and community empowerment.
- Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
- Proficiency in the use of computer technology and various software applications (PS Suite EMR an asset).
- Ability to speak additional languages is an asset (i.e., Amharic, Somali, Swahili, Tamil, Vietnamese, Bengali).
- Flexibility to work some evenings or weekends as needed to accommodate client schedules and community events.
- Commitment to valuing diversity, equity, partnerships and advocacy in keeping with Regent Park Community Health Centre’s Strategic Direction.
- Knowledge of the Regent Park area and issues affecting families living within the Regent and Moss Park communities
- Must be fully vaccinated for COVID 19, for the protection of clients and staff
