Nurse Practitioner, Interprofessional Primary Care Team
We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services.
We currently have partnered with the Downtown East Toronto Ontario Health Team, (DET-OHT) to develop and implement an Interprofessional Primary Care Team. The Interprofessional Primary Care Team (IPCT) is an initiative by the DET-OHT that brings together Regent Park CHC, St. Michael’s FHT, Inner City FHT, and Sherbourne Health FHT to enhance access to interdisciplinary primary care for individuals in the downtown east of Toronto, including Moss Park, St. Jamestown, and Regent Park, who face significant financial challenges, housing insecurity, and other forms of marginalization. The DET-OHT comprises 43 health and social service organizations deeply rooted in the community, aiming to provide seamless, inclusive, and high-quality care through an integrated system. The DET-OHT’s initiatives focus on priority populations, such as people experiencing homelessness and those with mental health and addiction issues, guided by principles of client and family-centered care, health equity, diversity, transparent collaboration, quality improvement, and responsive leadership.
Job Summary
The Nurse Practitioner will support client attachment to primary care services. The NP will work closely with the DET-OHT IPCT central navigation team and the four primary care organizations within the DET-OHT to deliver integrated services, with an emphasis on health stabilization and fostering strong, ongoing attachment to primary care.
The Nurse Practitioner will provide comprehensive primary care to a diverse population facing significant barriers to healthcare, assisting patients in managing chronic conditions, preventing illnesses, promoting health, and effectively navigating the healthcare system.
Three Year Contract Full Time – 35 hours per week
Salary Range: $112,012 to $134,936, plus opportunity to participate in HOOPP (Healthcare of Ontario Pension Plan).
This is a bargaining unit position within OPSEU Local 5115.

Providing Patient Care:
- Deliver comprehensive primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO), including care to individuals who experience acute medical issues, chronic health conditions and those who have mental health and/or substance use challenges. This will include: comprehensive health assessments, making diagnoses, monitoring ongoing therapy of clients with chronic illness, ordering diagnostic tests, interpreting results, and prescribing medications within the scope of practice of NPs.
- Ensure that standards of client care are maintained according to the Standards of Nursing Practice, the Standards of Practice for Nurses in the Extended Class, and generally accepted ethical guidelines for, as well as functioning within the Regulated Health Professions Act and the Nursing Act.
- Work closely within Regent Park CHC’s multidisciplinary team to provide primary care
- Collaborate with team members, agency partners, clients, and communities to plan, implement, and deliver low-barrier, client-centered and coordinated care.
- Perform outreach to marginalized individuals and groups, advocating for clients, and supporting community development.
- Work collaboratively with clients to identify and mitigate health risks, promote understanding of health issues, and support healthy behaviours.
Provides health promotion and education:
- Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs
- Advocate for individual client needs to ensure access to effective care, follow-up and resources from both an individual and systemic level
- Providing health information in an accessible, appropriate ways to community and clients through pamphlets, posters etc.
- Sharing in teaching and supervision of medical learners.
Advocates on individual and community issues:
- Participate in relevant meetings and actions as per internal policies and protocols;
- Participate in the development and/or support of policies and strategies which are in the interest of public health and healthy public policy, recognizing the social determinants of health such as adequate income, housing, and food
- Advocate on an individual basis for and with clients to other agencies and institutions
Is active in outreach and community development by:
- Participate in other RPCHC programmes and activities as appropriate;
- Participate in other community projects or agency activities as a representative of the Health Centre;
- Develop and maintain working relationships with existing community and professional groups.
Qualifications and Skills Required
- Current registration with the College of Nurses of Ontario as a Registered Nurse, Extended Class.
- A minimum 2 years of Primary Care NP practice or a new NP with 3 years of primary care/community RN practice within a relevant clinical role
- Demonstrated understanding of working within and anti-oppression framework
- Experience providing direct support to individuals from marginalized communities including those who experience homelessness, substance use, mental health challenges, poverty and those from racialized communities.
- Demonstrated ability to apply trauma informed principles in work with clients and community
- Experience in conflict resolution and crisis intervention
- Demonstrated commitment to client-centered, community-based care
- Experience working in community-based primary care
- Experience in program development, implementation and evaluation an asset.
- Experience and comfort working with and electronic medical record (EMR)
- Knowledge and proficiency in current, evidence-based methods and practices in healthcare delivery, with an emphasis on mental health promotion and risk reduction
- Working knowledge of relevant legislation, and professional standards that govern nursing practice, including the Personal Health Information Protection Act (PHIPA), Regulated Health Professions Act (RHPA) and the College of Nurses of Ontario (CNO) Standards and Guidelines
- Extensive knowledge and understanding of harm reduction principles and practices;
- Ability to work collaboratively within and across teams
- Strong oral and written communication skills
- Excellent leadership and planning skills
- Demonstrated interpersonal, decision making, critical thinking and organizational skills

- Knowledge of the Regent Park area and issues affecting families living within the Regent and Moss Park communities
