Our interdisciplinary clinical team of physicians, nurse practitioners, nurses, and clinical assistants provide holistic primary care to local community members.

Our services are grounded in the philosophy of client-centred care and the concept of individual and community empowerment. Clients within the clinic are assigned a primary care provider (physician or nurse practitioner), or a healthcare team, depending on their unique needs.

All providers work together as a team to provide the best health care possible which includes communicating with each other and consulting with other caregivers as part of the ‘circle of care’. This enables us to provide our clients with a broad scope of treatment.

We also offer urgent care (same day/next day) access for registered clients.

Our team also provides primary care in shelter settings.

Eligibility

We serve residents in the catchment area from south of Gerrard, north of King, west of the Don River, to the east of Sherbourne.

If you do not have any health insurance, the catchment area is from south of Bloor, north of Lake Ontario, west of the Don River, to east of Yonge Street.

We regret that we cannot serve individuals here on VISAs or work permits.
