We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, Pathways to Education, infant and child development programs, parenting programs, youth outreach and employment services.

Position Summary:

The Registered Nurse assists in the provision of direct patient care and community services in order to improve the health of the Regent Park Community. As a member of a multi-disciplinary team, working on or off site, the RN serves a high density low income, multicultural, inner city community through a variety of methods including direct client care, health promotion and education, advocacy, and community development.

This is a Full Time Permanent Position, 35 hours per week

Hourly Range: $34.71 to $ 41.82, plus Benefits including HOOPP Pension

This is a bargaining unit position within OPSEU Local 5115.

Responsibilities Include:

Provides direct client care by:

  • Triaging of walk in clients and telephone screening of clients.
  • Developing and implementing, with other team members, a plan of care for clients with stabilized chronic illnesses (e.g. diabetes, hypertension, obesity).
  • Screening and assessing clients with minor episodic illnesses according to established protocols.
  • Providing supportive and therapeutic counselling and crisis intervention on an individual or group basis on such issues as sexuality, family planning, pregnancy choices, family violence, and lifestyle.
  • Assists with referring clients to specialist, hospital or community agencies as needed after consultation with team members.
  • Visiting clients in their homes as required.
  • Performing basic nursing skills (e.g. dressings injections, suture/clip removal and assisting with other procedures as required).
  • Performing added nursing skills (according to College of Nurses) as required as venipuncture, pulmonary function testing, ECG’s.
  • Assisting physician(s) with prescription ordering.
  • Assessing and making recommendations for the purchase of equipment as needed.
  • Cleaning and sterilizing of equipment.
  • Maintaining accurate records.

Provides health promotion and education both to individuals and to groups in a non-judgmental fashion by:

  • Participating in Health Fairs or Community Fairs as required.
  • Providing medical information in an understandable way to the community at large through posters, pamphlets, community publications, etc.
  • Sharing of teaching and supervision of health care students.

Advocates on individual and community issues that are of concerns to clients and the community at large by:

  • Participating in relevant demonstrations and meetings as per internal policies and protocols.
  • Participating in the development and/or support of policies and strategies which are in the interest of public health, recognizing the social realities which influence health such as low income inadequate housing.
  • Advocating on an individual basis for and with clients to other agencies and institutions.

Qualifications and Skills required:

  • Certification as a Registered Nurse in the Province of Ontario.
  • Knowledge of the Health care system, social determinants of health and community resources.
  • Understanding of the patient as an individual and as a member of the community.
  • Wound care management and experience with overdose prevention and response, and training in/experience with Naloxone administration an asset.
  • Demonstrated commitment to client-cantered, community-based care.
  • Experience working in community primary care settings.
  • Experience using electronic medical records (PS Suites an asset).
  • Strong interpersonal communication skills.
  • Ability to work extended (before 9am, after 6pm, weekends) hours as required.
  • Demonstrated organizational and time management skills.
  • Demonstrated experience in program planning, research and evaluation.
  • Ability to work collaboratively within and across teams.
  • Understanding of Anti-oppression principles.
  • Ability to work effectively with people of various cultural, age and economic backgrounds.
  • Knowledge of the Regent Park area and issues affecting families living within the Regent and Moss Park communities
  • Must have up to date COVID vaccine for the protection of clients and staff
